was conducted on these three data sources and each of these ratings were combined into one overall rating.
When looking at the five-star ratings, more stars are better. Five (5) stars are themost a nursing home can get. One (1) star is the fewest.
About
our Intermediate Care Facility (ICF)
The Intermediate Care Facility at Harbor View House is an 83-bed unit
licensed by the Department of Health Services (Lic.#910000062) designed
for adults 18 years or older who have a mental illness and either a chronic
or acute physical illness. To qualify for intermediate care services,
a potential resident must have a medical condition which needs an out-of-home
protective living arrangement with 24-hour supervision and skilled nursing
care or observation on an ongoing intermittent basis to prevent deterioration.
ICF services emphasize care aimed at preventing or delaying acute episodes
of physical or mental illness and encourage each patient's independence
to the extent of the resident's ability. Examples of the conditions treated
on this unit are diabetes, high blood pressure, seizures and respiratory
issues such as chronic obstructive pulmonary disease (COPD) and asthma.
ICF fees can be financed 100% by the State Medi-Cal reimbursement system.
Private pay is also welcome.
Staffing
The ICF Administrator, Carla Duke Williams, M.A., N.H.A., has a master’s
degree in Gerontology and has been with HealthView, Inc. since 1997.
Christy Oh, R.N. serves as Director of Nursing (DON) and has over 15
years of skilled nursing experience. Our nursing staff consists of
a Charge Nurse, Licensed Vocational Nurses (LVN's), and Certified Nursing
Assistants (CNA's). Support staff include a Social Services Designee
who oversees each resident's psycho-social needs, podiatry, dental
care, and eye care and an Activities Coordinator who organizes the
ICF's daily activities schedule including daily exercise routines,
current events, table games, arts and crafts, special events and outings.
Several after-care options are available through Harbor View House Adult Residential Facility and Harbor View House's Independent Living Apartments.
